Each year you must have an authorization form to receive/keep and F.O.P. plate. You may apply for your plate anytime during the year, but please allow enough time for verification and mailing.
The following application must be mailed to the INDIANA F.O.P. STATE LODGE SECRETARY in order for you to receive an authorization form for the "Special Recognition Plates."
The plates are $25.00 a plate. A check for the correct amount must be submitted with the application. Please make the check payable to Indiana State F.O.P. Lodge.
The following application is for the Secretary's Office to verify your membership in the Fraternal Order of Police of Indiana. After verification an authorization form will be mailed to you. The authorization name must be identical to one name on the registration. That person must be a member of the Indiana F.O.P.
The authorization form must accompany your vehicle registration form when you purchase your plates.
